- Baptismal certificate (if applicable)
- Updated immunization form
- Dental screening form
- Physical form 

Nothing additional for Grades 1-5, as that information will come over in the student’s cumulative file.

- Tour / Meeting with St. Matthew principal (required only for students new to St. Matthew Elementary)
